FY23 Community Building Agreement Overview:

First Payment Requirements:

• Complete the FY23 Avondale Partnership & Investment: Community Building Technical Assistance Orientation

• Complete and return a FY23 Community Building Needs Assessment with refined project outcomes

• Sign and return a FY23 Avondale Partnership & Investment: Community Building Grant Memorandum of Agreement

• Provide an invoice for half the amount of the grant award with the expense noted as: FY23 Community Building Grant Payment 1 of 2 – Email to communityrelations@cchmc.org,  Subject: FY23 Community Building Invoice

 Second Payment Requirements:

• Complete and return a FY23 Avondale Partnership & Investment: Community Building Grant | Mid-Year Report

• Provide an invoice for half the amount of the grant award with the expense noted as: FY23 Community Building Grant Payment 2 of 2 - Email to communityrelations@cchmc.org,  Subject: FY23 Community Building Invoice

In addition, each grantee is required to comply with the following guidelines:

• Complete the project during the FY23 Grant Cycle: January 1, 2023 - December 31, 2023

• Submit Quarterly Reports to share financial and outcome progress on or before the following dates:

• Participate in three (3) Technical Assistance Trainings offered by Cincinnati Children’s Hospital Medical Center in partnership with subject matter experts.

• Grantees are highly encouraged to attend the All Children Thrive Learning Sessions (Fall/Winter 2022 and Spring/Summer 2023)

Of note, any proposed revisions to the Project or to the Project budget requires approval from the Avondale Partnership & Investment Committee. Noncompliance with the terms and conditions of this agreement or misuse of funds is cause for discontinuation of funding.

It is also important to cite the Avondale Partnership & Investment: Community Building Grant in marketing and communications related to outcomes achieved by this grant. Please use the following citation to reference the grant. In addition, the Cincinnati Children’s Hospital Medical Center, Community Relations Department would like to receive an electronic copy of corresponding collateral or communications.

Citation: “The project described was supported, in part by, the Avondale Partnership & Investment Committee. The content is solely the responsibility of the authors and does not necessarily represent the official views of the Avondale Partnership & Investment Committee, nor any of its members.”

Lastly, please note that Cincinnati Children’s Hospital Medical Center is net 45 from receipt of invoice.
